Certification of Completion: New Hire Safety Orientation

Strongwell has a longstanding commitment to high standards of team member health and safety. The safety of all teammates is very important to us and every new team member must complete the New Hire Safety Orientation prior to working in any manufacturing area of the facility. I certify that I have completed the New Hire Safety Orientation. I have been informed of Strongwell’s policies and procedures concerning the following topics:

• Workplace Hazards
• Personal Protective Equipment (PPE)
• Lock Out Tag Out
• Hazard Communication
• Back Safety
• Prohibited Tasks
• Emergency Procedures

I understand that it is my responsibility to follow all safety guidelines and company policies. If I have questions about safety protocols, it is my responsibility to ask my supervisor or a member of the EHS department. If I have any questions about company policies and/or procedures, it is my responsibility to ask my supervisor or a member of the HR department. I agree to make safety a priority every day.
